Asset-based
approaches are concerned with identifying the protective factors that
support health and wellbeing (McLean, 2021). It is argued that they
offer the potential to enhance both the quality and longevity of life
through focusing on the resources that promote the self-esteem and
coping abilities of individuals and communities. Asset-based approaches
emphasize the need to redress the balance between meeting needs and
nurturing the strengths and resources of people and communities (McLean,
2021). All people need to gain more control of their health is the
right resources. Some positive resources in the community is the WIC
Program. The WIC program is for pregnant women, infants, and
children (Special Supplemental Nutrition Program for Women, Infants, and
Children (WIC), 2020). If a mother or soon to be mother has low income
or no income to provide the healthy for necessities doe a growing baby
or child, they help provide it. They also help with other resources such
as breast feeding classes, wellness checks, nutrition counseling,
vaccinations and check infant and children for lead poisoning (Special
Supplemental Nutrition Program for Women, Infants, and Children (WIC),
2020). This resource can help provide education, and nutrition guidance
to the soon to be mother. Then in return they can utilize the resources
to take their health into their own hands and educate their children
about the healthy habits that they have learned. When cultural aspects
is be learned as nurses we have to be sensitive to our patients
culturally background. Nurse play a major and vital role in healthcare
needs of certain cultures. Some cultures have a certain time that they
need to eat, so the timing of a meal for this patient is
critical (Ferwerda, 2016). Sometimes even the gender of the healthcare
working with certain cultural patient need to be addressed at
times (Ferwerda, 2016). As nurses we need to make an effect to meet
cultural needs to the best of our ability but sometimes it may not
always be feasible. It is very important for nursing staff to understand
cultural needs, which will help assist a patient health outcome better.

Asset-based
approach to health improvement is a way to support good health and
well-being. It protects us from bad health and promotes a good community
to uphold these standards (McLean, 2021). These things help people who
are less fortunate or high-risk. This can refer to mental and social
health and the physical resources for the high-risk population. There
are three levels to the asset-based approach: Individual focuses on the
commitment to learning and self-purpose; community focuses on programs
within the community such as religious groups and family/friend
networks; and organizational focuses on employment opportunities,
volunteering, and having a safe house available (Rippon & Hopkins,
2015). Having the resources to stay “above water” during difficult times
is so important. Programs such as the Red Cross, local food banks, and
shelters are all examples. If a less fortunate person has clean clothes
to wear, food, and a roof over their head, it gives them the physical
and mental strength to continue to go on in life and provides them with a
safe place. Having the appropriate clothes to wear during a job
interview can help that person obtain a good job, then give them the
ability to provide for themselves and or family; the Red Cross can help
with clothing donations and local churches with soup kitchens or food
donation programs.

It is important to keep in mind the cultural differences while
providing care to patients. With learning comes a possible language
barrier that may cause a patient not to know or understand what is being
taught to them to improve their health. With particular religious or
food preferences in mind, there may only be certain food available from a
pantry that may go against culture. And in regards to clothing
donations, there may not be appropriate clothing for certain cultures.
In conclusion, with all the available help, sometimes there continues to
be a barrier to helping some.

My
hypothesis is that lowering nurse-to-patient ratios will improve the
retention of nurses and lower costs of health care. “Almost one out of
five new nurses leaves the profession within the first year of gaining
licensure as a nurse…one out of three leave the profession within two
years” (Carlson, 2017, para 12). Part of our problem is that our nurses
are so overworked and nursing is so stressful that the people schools
have invested in then are driven out of the profession. If nursing
ratios were improved, the job would not be so stressful, and nurses
would be more likely to stay, increasing the amount of people in the
nursing pool. Another studied showed that a 1:8 ratio was the least
expensive but had the highest risk of patient mortality–but it also
showed that lower ratios such as 1:4 became more cost-effective when
lower ratios shortened length of stay and hourly wages were also low
(Rothber, et. al, 2015). While the decreased hourly wage is undesirable,
I think many nurses would still choose that over the higher ratios. If
we were able to retain more nurses through safer ratios, we could
improve quality of care, reduce readmittance to the hospital, and
ultimately decrease the cost of healthcare while also being safer.

Is there a correlation between high nurse workload and omission in
nursing care? Nursing shortages have been fueled by a retiring
workforce, and an aging population. Factors contributing to high
workload include increased demand for nurses, low supply of adequate
nurses, and reduced staffing and overtime in order to cut costs.

According to (Griffiths et al. 2019. The association between nurse staffing and omissions in nursing care: A systematic review) research
has established associations between nurse staffing levels and adverse
patient outcomes including in hospital mortality. This has been used to
argue for increased nurse staffing levels, including mandatory staffing
minimums.

Omission in nursing care can also directly effect quality
measures negatively. It is difficult to research this data because often
times omissions to errors are not readily reported. High nurse workload
can also adversely effect patient safety, nursing job satisfaction and contributes to high turnover rates.

Providing adequate clinical staff to meet safe nurse:patient ratios
and staffing to the acuity of the patients would provide the staff the
support they need to give exceptional patient care as well as increase
patient satisfaction and quality measures. With lower workloads nurses
could spend more time assessing patients and improve documentation.

What governmental, institutional, or private sector policies could enhance patient engagement and activation?

Upon
researching this topic of patient engagement and patient activation, I
found a plethora of information that I was not aware of before. Patient
engagement is an umbrella term that includes patient activation as well
as the interventions behind encouraging a patient to a motivated
behavior (Hubbard, 2019) Patient activation entails the patient to take
initiative and manage their care.

This
is a measurable process that is assessed through the Patient Activation
Measure (PAM). This questionnaire simply helps identify how involved
the patient is in their care. In using this the provider can tailor
their teaching methods to fit the level of activated the patient is.
Should the person have a lower activation the provider can start slow by
giving educational tools that are appropriate in explaining the
importance of compliance with medication or even the general process
behind the disease.

I
personally liked the tool mentioned in an article on patient activation
by Sara Heath (2019) on clinician led patient education letters. The
letter included education on patients conditions and potential treatment
option. A personalized goal such as goal blood pressure (BP) for
hypertension patients was included on the sheet as well as their last
measured BP. In doing this, the patient can be activated to see where
they need to improve. They also could initiate questions to their
provider in seeing the treatment options available to their condition.
This was a great way to help activate a patient to improve their
outcomes!

There
are many other tools and techniques to try to increase patient
activation! The outcome is patient’s can comply with medical advice and
make improvements in their own health independently.
