1. Client E is a 30-year-old male who reports having changes in mood that seem to cycle weekly. One week he has lots of energy and sleeps very little and the next week he cannot drag himself from his bed and wants nothing to do with anyone around him. He reports some occasional irritability, but feels it is largely due to the increased energy he experiences. He describes his thoughts as “racing” and others have noticed that sometimes he cannot grab one thought long enough to continue a conversation clearly. During the interview, he appears very depressed and disheveled. When questioned about the last time he bathed, he was unsure how long it had been. He currently denieshaving thoughts of suicide. When asked about the times he has more energy, he indicates these times are “great and wonderful.” During these times, he meets lots of people because he goes out daily and interacts with others. The lack of sleep during these times allows him to get a lot accomplished, but he is concerned with how his mood and energy levels seem to plummet afterwards. The other thing he admits is he tends to spend a lot of money during these high energy times. This indiscriminate spending has led to some financial difficulties for him. When he feels more depressed he tends to eat a lot and lay around in his bed.2. Client F is a 28-year-old female presenting in crisis. Her mother has kicked her out of the house for violating rules that were set up when her mother agreed to let her move back in. The client is looking for help getting a place to stay. When offered the option of a shelter, the clientbecame very angry and demanded that the interviewer pay for a hotel room. When told this was not an option she began to question why the interviewer wouldn’t help her. After explaining that the interviewer was trying to help and offering to call a friend, the client indicated she didn’t have any friends anymore because they “all left her.” She describes her relationships as difficult and explains that people often leave her after they have a disagreement. She also reports she sometimes feels like she doesn’t know who she really is. When asked to clarify, she mentions that her interests tend to change quickly, to the point that others have noticed these dramatic shifts. She describes some mood swings, but these primarily occur when dealing with other people. She can be fine one minute and then going off on another person the next. She denies current suicidal thoughts, but admits to having had them in the past. She has also attempted suicide in the past. When pushed, she admitted these feelings tend to happen when people leave her.
